Ballarat Health Services is seeking a dynamic, energetic Regional Assessment Service- RAS Assessor to work in the Aged Care Assessment Service. Ideally you will be a Registered Nurse Grade 2.

Working within a multidisciplinary team you will be expected to facilitate and foster best practise and service delivery to our clients and their families within the Local Government area of Hepburn Shire.

The Regional Assessment Service (RAS) is a new role within Ballarat Health Services and will sit within the Grampians Aged Care Assessment Service. The Regional Assessment Service operates within the Local Government area of Hepburn Shire. The Assessor will play an important role, as a member of a multi-disciplinary service, in providing a Home Support Assessment service primarily for aged members of the community and their carers. The likely candidate will have demonstrated experience in aged care and have highly developed assessment and clinical skills.

The assessor’s key role is to:

  • Conduct Home Support Assessments to determine a client’s eligibility to receive Commonwealth Home Support Program (CHSP) services. Home Support Assessments are conducted face-to-face in the client’s usual accommodation setting.
  • Provide information and refer clients to services that are appropriate and available to meet their needs and preferences.
  • Work together with the client to establish a support plan that reflects the client’s strengths and abilities, areas of difficulty, and the support that will best meet their needs and goals. This will include the consideration of formal and informal services as well as enablement pathways where appropriate.
