Social Worker – Grade 2

An enthusiastic and experienced Social Worker is sought to provide services to subacute/aged operations at Ballarat Health Services. This position will initially commence in residential aged care, there will be flexibility to work in other areas if required.

Primary responsibilities include clinical skills and a passion for working in aged care, excellent communication and collaboration in pursuit of team and resident/family goals is essential. Primary job requirements involve the provision of clinical social work services for residents of BHS Aged Care facilities and their carer/families. A working knowledge of adjustment, complex supports needs, counselling, ageing and end of life care relating to transition to care is desirable.

You will enjoy a friendly place to work where there is a strong commitment to providing high quality evidence-based care. There is excellent support from within the senior and broader Social Work Team. Professional development is supported through regular in-service education, supervision in line with AASW requirements and the ability to attend continuing education where appropriate.

  • Sound knowledge in Social Work theory and practice skills
